*I say while watching the intersection to make sure it's clear*

*After a couple near misses I see the girlfriend waving us down*

'Looks like that's her'

*ONce the girlfriend shows us where her boyfriend is*

'Richardson, it doesn't look to serious do you want to splint or get vitals?'

*Takes a look*
'Meh, we will just split it'

'Ok, im going to be 2 minutes while i run back to the ambo, what were going to do is, split your leg, and strap you up to the carry chair and get you into the warmth'

'kk, thank you soo much'

*Runs back to ambulance, grabs 4 heat retaining blankets, my spare jacket, 3 blankets, splints, takes down carry chair from the back of the door, and places all the equipment onto the carry chair*
*Remembers to turn on heating for the back on ambo*
*Wheels everything to the patient with difficulty*
*Gives his girl friend a blanket and my spare jacket*
'Here have this, its my spare'

'Thank you soo much'

*Gives splints to Dave, while i set up the carry chair*

"BLS 1 to COPA, advise hospital for a spare bed in A&E, over"

*Wraps patient up in blanket, while Dave puts on the splint*

'There you go'
'you need any help Dave?'

'No, i'm just about done.'

*As I finish putting the splint on*

'And hold on for a second, i'm gonna get this guys vitals'

'BP: 110/70 P:98 Resp: 20/Regular'

'I'm gonna ride in the back with the patient, you drive'

*As I stand up, the patient's girlfriend walks back up*

'Is it ok if I ride to the hospital with yall?'

'That will be fine.'

'If I can get you to ride in the front with Richardson.'

'Ok, thank you'

*After getting the patient settled on the cot I tell Richardson were ready to go and advise dispatch*

"Basic 1 to dispatch: Be advised we are enroute to EDC non-emergent"

'Ok'
*Waits until he finishes assessments*
*Helps up patient and sits him on the carry chair and straps him up tight*
*Wheels back to ambulance*

'The back of the ambulance should be warm for you, ok mate?'

'Yeah thanks'

*Hands over to Dave*

*Talks to girl*

'Hi, my names Vaughn, you don't need to worry about a thing, my driving is GREAT'

'Ooh Hey! Ok, i trust you to get us there safely' *she laughs*

*I laugh*

*Turns off grill lights and rear blues, and sets off for hospital*
*Pulls out from a slight ditch and stalls the ambulance*

'Grrr, im sorry, this is first time ive drove this thing' *giggles*

*shes silent*

'Sorry back there!'
*Turns everything off and starts again*
*Revs up a little, and pulls out of ditch*
*Sets the Sat-Nav to Hospital, and drives*

'Shouldn't take too long, its not that busy today'

'Ok'

*Pulls up at a lighted junction, waiting for red light i ask girl*
'Are you cold?'

'No im pretty warm, these foil blankets are pretty good'

'Yeah i know, its good thing i packed them in before we left the station'

*Proceeds to hospital*

(American ambulances, well ERT ones don't run blues fyi)
"COPA BLS1 status update please"

*After falling off the bench when the ambulance stalls*

*I start laughing and the patient starts to chuckle*

'Sir, I need to get some information for my Prehospital Care Report'

'Ok'

'Whats your name?'

'William Boswell'

'Date of Birth?'

'July 21, 1995'

'Do you have any pre-existing medical conditions?'

'No'

'Allergic to any medications?'

'No'

'Ok, that's all I need for now.'

*After completing the PCR, I stand up and move over to the captains chair and pic up the microphone.*

"Basic 1 to EDC"

"Go ahead Basic 1"

"Afternoon EDC this Lafferty and Richardson on Basic 1. Were enroute non-emergent with an ETA of about five minutes. Patient's Chief complaint is a broken leg, vitals are BP: 110/70 P:98 Resp: 20/Regular, Interventions are leg has been splinted distal pulses present and strong. Any furture orders,questions,comments?"

"Negative see you in five"

*Arriving upon the hospital, i turn on the back reds to alert traffic of our presence*
*Unlocks all doors from the automatic lock, and advises girl to come to the back of the ambulance*
*Jumps out of ambulance and bangs on side of ambulance to let dave know we're here*
*Opens up back door and smiles*

'Everything ok?'

(This is 2011, he'd be 20, not 15, good try though XD)

'Ok Mr. Boswell were going to get you in the hospital and have your guardian come down here'

'Ok, Call my mom this is her number ***-****'

'Ok, sure thing.'

*As we are walking in the ER entrance, I see a nurse standing by the door*

'Hi, this is Mr. Boswell.'

'He has a broken Tibula, which has been splinted and distal pulses present and strong'

'Here is his paperwork, and this is his mothers number. If yall could call her down here.'

*After the nurse signs my PCR transfering care to the hospital I then file the PCR and advise dispatch*

"Basic 1 to dispatch: Be advised we are back available and enroute post"

*After saying good bye to The casualty and his girlfriend, i tidy and clean the back of the ambo, putting equipment back and wiping down the stretcher*

*Locks back door, and jumps in drivers seat*

*Turns all lights off and drives back to the station*

*On the way, i quickly stop at a cafe and buy 2 cookies and 2 hot chocolates for me and Dave*

'I'll be 2 minutes just going to get refreshments'

*gets back in ambulance and puts drinks and bag on the side*

'thats for you and theres a cookie in the bag, you eat mine and you will need me to take you back to the hospital'

*I laugh and drives back to station*

(We can now head over to the Break Room in the EMS/Hospital section to continue )

*I laugh as he hands me my cookie and hot chocolate*

"ALS-1 to a possible stroke on the end of Plum way. First responder medic on scene prvoiding CPR"
EAT:
72 YO male stroke reportedon Plum Way, first responder on scene, no more information given.

* Hearing the call come in I turn on the lights and siren and head off into traffic *
' ALS-1 Copies responding, 4 minutes out. '

* We pull up on Plum way and are quickly flagged down by a person on the street. *
' COPA, ALS-1 on scene we have 1 person down ont he sidewalk, respond 1 PD unit to this location for questioning and reporting. '
* I get out and grab the ALS bag from the side compartment and quickly walk over to the patient. *

I get out of the rig and grab the defib/moniter. I walk over to the patient and check pulse and resperations.
"CARDIAC ARREST" I yell
I rip open his shirt and quickly attach all the leads of the ECG.

* As I reach thepatient I observe severe left side facial drooping, I notice there is a person giving CPR already, Zach starts to get a set of vitals while I walk over and assume CPR from the bystander. *
' Sir i'll take over CPR from here in 3,2,1 got it. '
* the bystander moves off and i assume the compressions. *
' COPA, ALS-1 we have a male approx 72yrs old in full arrest, left side facial drooping is observed, patient is diaphoratic, pupils are non reactive, Stand by for further vitals. '

"Pulse 0 Resperations 0 BP 0/0 Pulse Ox 0%, pulse present with chest compressions and sir can you remove the BVM so I can intubate" He let's me intubate and I insert the tube through the vocal cords. I have the EMT who's bagging him continue. "Lung sounds good, No stomach sounds ET tube placement good" I get out an IV and insert the IV needle and attach the IV bag to it.

' Any other people here certifiedto do CPR?, I need a person to take over chest compressions while I go get our stretcher. '
* 1 man raises his hand and introduces himself as a Paramedic from Minnesota, He takes over compressions for me as i get up, run to the ambulance and take out the stretcher and wheelit over to Zach, I press the button on the bottom of the handle and lower it to the loading position. *
' Zach, you wanna load him now? '

"Yeah" I put him on the sterecher and get our supplies back to the ambulance.

* I raise the stretcher and startmving him towards the ambulance where Zach helps me load him up, I close the doors leaving Zach to ride with the patient to the hospital and start walking back to the drivers door *
' Thank you guys for your help tonight we really appreciate it. '
* I get back int the ambulance, and turn the lights and sirens back on and slowly pull back into traffic *
' COPA. ALS-1 en route Code 3 to hospital with 1 patient. '

"COPA to BLS-1. Respond to 178 Keller Street, cross is North Twin Road, for the fall victim."

[EAT]
178 Keller, fall victim. Caller advises 37 y/o male fell down stairs, victim is alert and conscious, reporting pain in the left leg, is unable to walk on it. Caller advises to enter through side door by the driveway.

*Upon hearing the tones on my handheld I grab my jacket and head out the door*

"Basic 1 copies"

*I say as I step in the truck*

*I then put my seatbelt on and start the truck.*

*As I pull into traffic I say into the radio*

"Basic 1 is enroute to 178 Keller"

*After an uneventful trip we arrive on scene*

"Basic 1 is on scene"

*I grab a pair of gloves and put them on as I go to get the cot*

'Richardson lets go ahead and get a spine board in case we need to use it to get the patient on the cot'

*As we walk in the side door I call out and hear the patient yell that he is in the hall*

'Hello sir were EMT's with ERT.'

*The patient states his name to be James Williams*

'Ok Mr. Williams can you tell me what happened?'

*He states that he was walking and tripped and now has pain in his left leg*

*After exposing his left leg I note that he has a deformity to the Tib/Fib*

'Alright sir were going to splint your left leg then we will transport you to the hospital'

'James: Ok, just be careful it hurts'

'Me: We will try our best, but you might experience some pain'

*After Richardson puts on the splint we put the patient on the cot*

'Richardson if you would drive, i'll ride in the back.'

*After loading the patient in the ambulance I get vitals*

'BP: 120/80 P: 100 Resp: 20'

'Ok Richardson were good to go'

"Basic 1 to dispatch: Be advised we are enroute non-emergent to EDC"

"SEV-1 please respond to the Phoxlind mall to assist with a rescue. PEMS is on scene requesting rescue support for an eldery female who has fallen on the escalator and has been trapped when it took in her dress. They'll need some help there please."

(I thought we were going with the other call I guess its only SEV 2)

*I grab the mic and throw my gear in the back of the truck*
"Sev-1 in route responding code 2"

(Fire support, not Rescue Support, you're rolling on this one)

(Yeah i edited and this is sorta what I meant but I meant more of our people requesting us to help them but this works to haha either way this small calls should be fun to. Thought I'm not sure why they don't just cut her dress I'll have to wait till where on scene to understand.)

*I pull up outside the mall and park in the fire and leave it running with the warning lights on*
"Copa this is SEV-1 show us on scene were going to be in the mall unsure if you'll be able to reach us. I hit the lock button as I hop out and grab the extra set of keys. Then I turn to mitch*

'Hey lets leave our gear here we shouldn't need it and I'm not quite sure how shes stuck so lets not take anything in just yet.'

'Alright, I'll take the ALS and get a rough assessment on her condition. Then we will figure how to get her out. Sound good?"

I open the compartment where the ALS bag is kept. I pull it out, fling it over my shoulder and walk into the mall with Moon.

'Sounds good but PEMS is already here they should have treatment handled'
*We walk into the mall and towards the escalators where I can see a large crowd has gathered despites mall security trying to move them. I fight my way past the crowd*
'ERD:FD make way people'
*As I get to the other side I see its a little more serious then it sounded on the radio it seems her leg has somehow been partially lodged under the steps*
(I'm not quite sure how else someone could get caught that bad in an escalator)
'Oh wow uh Mitch see if they need you for medic support what do you think we should grab to cut that step without making the injury worse?'

'I'm not sure let me find the maintenance man I don't know much about an escalator'

*I walk over to a man with dirty overalls*

'Hi there is an access panel to get under this right'

Man: 'Yes sir'

'Okay good can you open that up for me'

Man: 'Yeah sure'

*I follow the man to the access panel and he opens it for me*

'Okay thanks'

*I walk down the steps to size up the situation*

'Hey mitch you might want to some down here we got some blood you can bandage up. I think if we get the sawzaw down here I can make a couple cuts minimize damage to the escalator and the patient I'm going to go grab that'

*I walk out to the truck to grab the sawzaw and send another message to COPA*

"COPA this is SEV-1 out at the mall we are attempting extraction heading back in the mall may be out of radio contact"

*I head back into the mall to check on mitch*

'Hey hows that bleeding coming?'

((Allright))

I set down the ALS bag and start treatment on the lady's bleeding leg. The cut is pretty shallow and is not bleeding much. I grab the compression bandage and wrap it around her leg. Once that is secured I decide to leave the rest to the PEMS crew. They should have it under control. I walk over to Moon to discuss how we get her leg out.

'Uhh your the medic take the sawzaw and i'll do what ever you say since you probably know how best to cut this off without hruting her'

*I hand him the sawzaw*

"Alright, the best way to get her out is to cut the leg out and still leave some steel around her leg. Once we free her and she is on the stretcher we will get the rest of the steel off her leg."

I grab the sawzaw off Moon and start to cut around her leg. I yell out to her: "Hows the pain?" She replies that it is alright. I continue and finally get all around the leg cut out. Me and Moon walk out of the repair bay and walk to where the lady is. Me, Moon and the PEMS crew lift her up onto the stretcher and get her settled. She tells me the pain has got worse and that she would like some painkillers. I go to the ALS bag and grab the Penthrane and grab the green dispenser. I put 3 drops of it in the dispenser and tell her to suck on it when ever she fells pain. As she inhales on the pain relief I set to work on cutting the rest of the metal of her leg. I slowly cut down until I reach the end of the metal. I pry the metal open then carefully remove her leg. I stabilize her and hand her off to the PEMS crew. They wheel her off to their ambulance and I grab my COPA radio:"COPA, this is SEV-1 we have freed the woman and she is how in PEMS care. We will finish up here and return to the station. SEV-1 out."

*I pick up the sawzaw and follow mitch out to the truck and put all the gear away. Then I hit the unlock button and hop into the drivers seat as I pull into the parking lot to head back to base I flip the warning lights off and grab the mic*

"Sev-1 Clear of the mall heading back to the station"

"Central to ALS-1. Respond to the Country Club on the Trulit Interpass, in the male's locker room, for the CPR in progress. Caller advises country club security is on scene providing CPR, I advised the caller to have somebody look for an AED. Best access to the locker room will be in the rear of the building."

* Hearing the call come in, I quickly pay for my soda and rush out to the Ambulance. *
' COPA, ALS-1 responding to the Country Club. '
* I put the ambulance in gear, turn on the lights and sirens and drive off towards the Interpass. *

* I pull up to the country club and turn off my siren while on approach to the assigned building. One of the Security guards comes running up the ambulance and waves frantically to us. *
' Help, Help Over here. '
' Me: Whoa Whoa let me stop the truck first. '
' Guard: There is no time please hurry. '
' Me: Ok Ok we r on our way there. '
* I stop the ambulance and I hear Zach on the radio *
' COPA, ALS-1 on scene. '
* I quickly get out and Zach and I grab the Stretcher loaded with the ALS bag, O2 and Defib and walk briskly over to the patient. *

* We walk up to the patient and unstrab our gear from the stretcher, I go over to the patient and relieve the Security officer from CPR. The officer looks beyond exhausted. I continue to do CPR while Zach hooks the leads up to the patient for the EKG. *
* I Look over at Zach who quickly turns on the EKG *
' Zach: His waves are all over the board Looks like PSVT, charging Defib to 150. . . Clear. '
* I pull off the patient and Zach hits him once with the defib Converting him, I begin to ask questions of the people around me while Zach continues to work on the Patient *
' Is he related to any body here? '
* 2 ladies come forward and introduce themselves as his sisters *
' Does he have any history of Heart problems? '
Sister 1: Yes he does he has had 2 heart attacks before '
Sister 2: No he hasent he has only had 1 Dont you remember the other 1 was ohhhh what is that thing when you dont digest something right '
sister 1: Oh right right right I remember that day werent we at the restaruant, what was the name of that place. '
Me: Excuse me ladies lol but do either of you have a medication list or know off hand what he takes?. '
sister 2: No i dont but im sure he takes that medicine, hmmmmm you knwo I cant quite remember it now but It will come to me. '
Me: Ok Nevermind we are taking him to Memorial hospital now so you can meet us there. '
* I get on the radio *
' COPA, ALS-1 We have 1 Male patient Approx 71 years old with what appears to be a Heart attack, CPR was initiated but stopped when the ECG Showed Supraventricular Tachycardia. We converted with 1 shock at 150 patient is Sinus Tachy now, Unknown on med list at this time. We will be transporting shortly, Over. '

"Received ALS-1, you're out."

' ALS-1 Copies COPA. '
* as im finishing up on the radio Im repeatedly tapped on the shoulder by one of the sisters. *
ME: yes?
Sister 2: you know. . . . Now that i think of it I do rememebr it now, we were at a Pancake house and he made the comment that his chest hurt and he started Drooling. '
ME: was this recently?. '
Sister 2: Oh god no this was like 7 or 8 years ago. '
ME: Oh my lord well that is fine but Im more focused on what happened today.
Zach: ' Jamie lets move '
ME: ' alright '
* We hoist the patient onto the stretcher and load him onto the ambulance. I step into the back of the ambulance with the patient and Zach closes the doors and starts to drive us to the Hospital. *
' Jamie code 2 or 3? '
' 3 please Zach '
* As we start rolling towards the hospital I start an IV of D5NS wide open and continue to take vitals. *
' COPA, ALS-1 We are en route to Memorial Code 3 with 1. '
